Hopes and Dreams

Transitions’ Children’s Program addresses needs of children, adolescents and young adults with developmental disabilities, functional needs and problem behaviors ages three to twenty one to ensure that they have the opportunity to learn, grow, and participate with their families, friends, and communities.  We work with children and families as they identify their dreams and then assist in setting and attaining goals to reach those dreams.

Values and Outcomes

Children are taught skills through practice and application.  Behaviors are managed through prevention (of challenging behavior), teaching (positive alternatives), and responses (to inappropriate behavior).  Progress is routinely reviewed and programs are modified when needed to ensure that participants are making forward progress.  Participant’s experiences of personal improvement, greater choice, self control, and happiness are the ultimate measures of quality.

Aspirations and Aims

Children’s services are coordinated with family, school, and other organizations to provide participants with real world opportunities to develop specific life skills.  Through our program, children will develop and improve their abilities in the areas of mobility, self-care, learning, language and communication, social interaction, self direction, community participation and safety, and preparation for employment.  All learning opportunities assist participants with accomplishing goals specific to them and their family.  

Progress and Advancement

Transitioning out of services occurs when participants meet their goals and no longer need help with day to day activities and tasks, similar to their same age peers.  Depending on the severity of need and/or disability, some participants will gain independence and move on, where others will receive fewer and fewer services over time.
