Our Services

Transitions Children’s Program provides home and community based, individualized services to meet objectives designed to meet the needs of each participant.  Services are also provided in our center when it is appropriate for the participant.

 

Purpose of Services

​

Services are designed to increase autonomy in daily living skills such as hygiene, communication, social interaction, and other areas to increase the participant’s ability to care for themselves – this occurs by teaching needed skills and managing challenging behavior that is interfering with the participant’s independence.  Services also assist families with setting up routines, having consistent expectations, and addressing behavioral concerns when the participant is with their family.  Services also increase the participant’s ability to participate in their community.

Support Services

Support services (Children’s Developmental Disability Support Services or Waiver services) are accessed when a child has a diagnosis of a developmental disability and shows a need for assistance per the program's guidelines.  Support services include Respite, Community Based Supports (CBS), and Family Education.  CBS is the most commonly utilized Supports service.

Intervention Services

Intervention services (Children’s Habilitative Intervention Services or CHIS) include Behavioral Intervention, Habilitative Skill Building, Interdisciplinary Training, and Crisis Intervention; the most commonly accessed services are Behavioral Intervention and Interdisciplinary Training.
